I'll warn you, I am new to this viewer as well.
Have you checked to see if you are using the correct .swf?
There are a few:
pano.swf - main application file, Flash Panorama Player.
pano0.swf - the same as pano.swf, but it uses different panoSuffixes parameter.
panoStripe.swf - the same as pano.swf, but it uses different panoSuffixes param
See /tutorials/files.html - for example, I use the pano0.swf because this one is set to handle image with _0.jpg, _1.jpg, _2.jpg, _3.jpg, etc. and this is the format that Pano2QTVR outputs cube faces. So I rename this file when creating my presentation.
Maybe this helped?

Thanks Kevin,
English is not mother tongue, I kept in mind only: same as pano.swf.
Let go deeper.

Same problem on MaxOSX10.4.10. In the pano.swf info-window (command-i) I should normally be able to set which application should open this kind of file. Many files are open as recommended, but neither Safari nor Netscape. When I do open for "All applications" I can select Safari, but get the message "loading: failed" as you do.
What is wrong?
Lots of issues here by me as well, although I see a lot of questions, & no answers.. a Bit dissapointing.

everything has been working perfectly for me with os x 10.4.10, if i open a blank page in safari, with no url loaded, and drop pano.swf on it, it loads just fine -
add the appropriate xml parameters to the url, and you can load your xml file this way as well -
